This page is designed to answer commonly asked questions regarding the proper disclosure of information.
- Who can receive health information?
Anyone with proper authorization from the patient, parent or legal guardian.
- How can someone obtain a copy of his/her health information?
The patient, parent or legal guardian must sign a Release of Information form that authorizes Palm Beach Gardens Medical Center to release copies of his/her records to whom he /she has authorized. You will need to provide a photo ID.
- What is the turn-around time for requested information?
Information will be mailed out typically within 3-5 business days.
- How would a Primary Care Physician obtain a copy of a patient’s record?
The primary care physician may always request these records directly from Health Information management. Otherwise, complete the authorization form with the physician’s information that you would like us to send them to.
- Is there a charge for copies of the record?
There is no charge if records are sent directly to your doctor, please make sure you list them as a recipient. Under Florida State guideline (code 59R-10.005), if you would like your records for your own personal use, there will be a charge of $1.00 per page for the first 30 pages and .20 cents per page thereafter.

- What do I do with the completed forms?
You may fax the forms to 561-694-7298 or mail to Palm Beach Gardens Medical Center (Medical Records, 3360 Burns Road, Palm Beach Gardens, FL 33410.
