AFib Cardiovascular Nurse Navigator

Palm Beach Gardens Medical Center patients can take comfort in knowing that our AFib Cardiovascular nurse navigator will partner with your primary care physician and/or electrophysiologist to guide you through our AFib program and ensure the management of your care is efficient, compassionate and individualized. From diagnosis through treatment, rehab and follow-up, all of your cardiovascular care needs can be provided at Palm Beach Gardens Medical Center.

The AFIb Cardiovascular Nurse Navigator assists with:

  • Guiding patients through the healthcare system
  • Advocating on behalf of patients to enhance their communication experience with healthcare staff and providers
  • Working with patients to find the resources and support they need
  • Coordinating services through the continuum of cardiovascular care
